Most providers submit claims to Principal on your behalf. Principal encourage this as it ensures the best and most accurate submission of your claim.

Vision
Locating an In-Network Provider
With the Principal vision plan through VSP, you may visit any vision provider. However in order to maximize your vision benefit , it is recommended you access participating providers by visiting principal.com/vsp.
Contact VSP
Call VSP customer service at (800) 877-7195 for vision benefit information. Please use name and date of birth to verify benefits with VSP while waiting for your permanent ID card. Benefits cannot be verified correctly with a Social Security number.
Submitting a Claim:
Managed Care Vision (VSP® Vision Care)
In-network: If you visit a VSP provider, you don’t need to submit a vision claim. The VSP provider submits claims on your behalf.
Non-network: If you visit a non-network vision provider who doesn’t submit claims on your behalf, you can submit a claim online or download and mail the VSP member reimbursement form (above). View VSP claims and reimbursement FAQs. If you need assistance, call (800) 877-7195.
Life Insurance
Life Insurance Continuation of Coverage
In the event of termination of employment, you have the option to continue your life insurance coverage if certain conditions are met. Should you wish to apply for continuation of coverage you must contact Principal within 31 days from the date your group life coverage terminated or reduced. .
