Program Overview

Eligibility

New Hires:

  • Employees are subject to a 30 day orientation period before the benefits waiting period begins. After the orientation period has been completed, employees will be subject to a benefits waiting period of 1st of the month, following 60 days. You will be eligible for benefits on the 1st of the month, following 90 days of employment. 

Dependent Child(ren):

  • Eligible child(ren) include:
    • Natural child, adopted child, step child, or child for which the employee has legal guardianship. 
  • Dependents are eligible until the end of the month in which they turn age 26 (unless disabled).

Spouses / Domestic Partners:

  • Only legal spouses and legal domestic partners are eligible (as defined by Tennessee law).

New Hires

Benefits Enrollment

You can enroll in your Benefits one of two ways:

  1. Call the American Exchange Benefits Service Center at (866) 824-3572.  Service Center hours are Monday – Thursday from 8:00 a.m. to 6:00 p.m. and Friday from 8:00 a.m. to 5:00 p.m.  You can speak with a trained Benefits Specialist who will answer your benefits questions, handle on-line password resets, and complete your enrollment.
  2. Online with Zevo Benefits.

    Click on “Get Started Now” to begin. You will be prompted to enter your e-mail address on file, the last four digits of your Social Security Number, and your Date of Birth. The system will identify you by these credentials and allow you to create a password. Once you have created your password, you will be able to complete your enrollment. Complete your benefit elections by clicking “Begin Enrollment” and following the prompts. For future logins, your User ID will be your on file email address.

After you have completed your elections, you will receive a confirmation statement for your benefit elections.

Dependent Audit

American Exchange takes pride in offering a benefit plan that ensures employees and their families have the best quality care, while keeping your premiums and out of pocket costs as low as possible. This process helps ensure that your premiums aren’t inadvertently spent on an ineligible member.

  • Only legal spouses and registered domestic partners are eligible for coverage. 
  • To be eligible, your spouse must not be eligible for coverage through their employer. If medical coverage is available for your spouse through their employer, your spouse must obtain coverage on that plan. If you enroll for family coverage, medical costs for your spouse may be denied if it is determined that your spouse has coverage available to them through their employer.
  • Children are eligible until their 26th birthday. 

Following Enrollment...

  • Following enrollment, you will receive an email request from the Benefits Service Center requesting the verification documents
  • Documentation due within 30 days of your enrollment deadline

Qualifying Life Events

No enrollment changes are allowed to your benefits during the plan year, except in the case of a Qualifying Life Event. 

Qualifying life events that can result in changes to your benefits coverage include, but are not limited to:

  • Marriage or Divorce
  • Birth or adoption of a child
  • Loss of a dependent
  • Medicare entitlement
  • Change in spouse's employment that affects benefits
  • Loss of other group coverage

 

Qualifying Life Events:

If you have a qualifying life event, notify the Benefits Service Center of the event and your new elections.  You must also provide the necessary supporting documentation to the Benefits Service Center via fax at (866) 597-2202 or via email at help@americanexchangebenefits.com within 31 days of the change. If you do not do so within 31 days, you must wait until the next open enrollment period to make any benefit plan changes.
